Medication therapy management, also referred to as MTM or medication management, is a term used to describe a wide range of services provided by Pharmacists, the medication experts, on the health care team. This type of service provides a patient with the unique ability to work directly with their Pharmacist to manage their medications and optimize therapeutic outcomes.
How Medication Management Works
The Pharmacist gains access to the patient’s medical history and reviews their current course of therapy. Pharmacists start by checking for drug-drug interactions, drug-disease interactions, and most importantly the indication for the medication being prescribed. Patients who use prescription medications, non-prescription medications, herbals, or other dietary supplements can greatly benefit from medication therapy management services.

At Injured Workers Pharmacy, our Pharmacists can obtain full medical records from your doctors' offices, including allergies and indications. Our Pharmacy system allows us to input all medications, whether filled with IWP or locally, to check for any potential drug interactions.
Once we determine that there are no drug-drug interactions, the Pharmacist checks to make sure there is no therapeutic duplications or possible optimizations of certain medications or regimens. If a patient has more than seven (7) medications, then he or she has access to our Complex Care Patient Program, allowing our team to monitor medication refills, prior authorization requirements, and any doctor calls required for the patient.
